CHAPTER ONE
ASSESSMENT OF PATIENT AND FAMILY
Assessment of patient and family is the first and vital step in the nursing process. It is concern with the systematic and continues collection validation and communication of client data from various sources with the purpose of the nurse establishing a data base about the client particulars, family’s medical and socio-economic history patient’s lifestyle and hobbies, patient past medical history.
The common tools used in assessment include interview, observation, investigation and records. This provide a systematic format upon which the nurse analyzes patient problems to come out to the patient of an appropriate intervention.
PATIENT PARTICULARS
Emmanuel Adjei, is a 32 year old young man born on 19 January,1978 at Akuse, in the Eastern Region of Ghana. He is from a family of seven (7), one female, six males. He is the third child of Mr. Kwame Adjei and Dzatu Adjei and hales from Agbledome in the Keta District of Volta Region. He reside at Kojokrom,a suburb of Sekondi-Takoradi, in the Western Region. He speaks Ewe, Fante and English. He weighs 53 kilograms 165 cm tall and fair in complexion, he is married with three children, two males and one female. Mr. Adjei is a welder. He worship with Seventh Day Adventist church with his family at Kojokrom.
FAMILY’S MEDICAL AND SOCIO-ECONOMIC HISTORY
According to Emmanuel, none of the family members suffer from any chronic illness and there is no known hereditary disease in the family. They normally experience headache, abdominal pain and common cold, which are normally treated with over the counter drugs. Mr. Adjei is a self employed young man and earn his income from his work as a welder. He has his own shop and four apprentices
