also the questions on that form are actually illegal as they are deemed fishing for private info and considered an attempt to steal people identity according to the FBI and SEC etc... who in their right mind are going to give YOU troy reid copies of their driver license front and back and a current photograph and their email and physical home address etc... proof you are trying to steal identities!
