I thought that their main source is inflammation.
It is already known that inflammation accelerates aging and also known that ROS are used to fight infections, either as external disinfectants (such as H2O2 solutions) or internally, by the immune system.

Another question is does stress directly increase formation of ROS, or not?

It's the other way round. An increased formation of ROS causes stress.
