About 2 hours later, when they knocked on the door, with the typical phrase used in my unit, "The patient has arrived", a patient of approximately 80 kilos, 1.72 cm in height, was admitted with a blow to the right parietal frontal region, that is to say, to the right side of the head. From the front to the back, with a hematoma in the right eye and a great edema in the back, with a deterioration of the neurological state, that is, he was already entering a stage of disorientation, he could not obey simple orders, he completely lost his muscle strength, and he could not open his eyes spontaneously, which is why, the on-call intensive care physician decides to proceed with oropharyngeal intubation, that is, to place a tube in the mouth, which goes to the lungs, so that he can breathe through a mechanical ventilator. This equipment would provide him with assisted breathing, considering the fact that after the intubation, the coma would be induced.

Al siguiente día, luego de lograr estabilizarlo, y proporcionarle una sedación eficaz que me permitiría brindar algo que en UCI llamamos Neuro-protección, se toma la decisión de realizar una TAC ( Tomografía axial computarizada) con el objetivo de lograr descubrir a que nos enfrentábamos, para nuestra sorpresa, no tenia lesiones o daño en su Cavidad Abdominal, sus Pulmones no tenían lesiones, cosas que suelen ser común en este tipo de pacientes como consecuencia de lo fuerte del accidente automovilístico que tuvo, sus Miembros Superiores estaban excelente, no había fractura ni lesión que causara un peligro para su vida, de igual manera en el Miembro inferior izquierdo estaba conservado, sin embargo el Miembro inferior derecho si se pudo evidenciar una Fractura Simple de Tibia y Peroné,


The next day, after stabilizing him and providing him with effective sedation, which would allow me to provide something that in the ICU we call Neuro-protection, the decision was made to perform a CT scan (Computerized Axial Tomography) with the objective of discovering what we were dealing with. To our surprise, he had no injuries or damage to his abdominal cavity, his lungs were not injured, something that is usually common in this type of patient as a result of the severity of the car accident he had, His upper limbs were excellent; there was no fracture or injury that caused danger to his life, and the left lower limb was also preserved; however, the right lower limb did show a simple fracture of the tibia and fibula, something very easy to resolve. However, the tomographic findings in the region of the skull were very alarming, worrisome and with a very poor clinical prognosis; a fracture of the base of the skull was found, in addition to a right frontoparietal fracture, and an intraparenchymal hematoma, in simple words 35% of his skull was fractured by the impact his head had against the pavement, in addition to a large intracerebral hemorrhage that resulted in a huge, a gigantic clot, this hematoma was a risk to his brain, it is important to highlight a situation that perhaps very little is known, but the brain is one of the organs of the body that can not have direct external contact with blood, In other words, blood can only reach the patient through the arterial and venous packages, in simpler words, through the veins and arteries. When there is direct contact between the brain and blood, an injury is produced that is called ischemia, which is defined as death of the tissue, by compression, by failure to provide oxygen, among others; Almost immediately the Neurosurgery team is contacted, the case is presented, and a request is made to take the patient to the operating room as an emergency, not 30 minutes passed, when we received a call from the operating room, informing us that ward number 3 was ready to receive the patient.

His clinical condition had become something urgent, every minute was important, it was something that is called in the health area as golden minutes, every minute that the hematoma passed caused more compression to his injured brain and decreased the oxygen supply that reached it, he was admitted to the operating room at 11: 35 am, with a bad prognosis given the size of the hematoma was approximately 15cm, everything ready, organized, surgeons ready, anesthesiology team ready, critical area nurses ready (this time I was in charge of maintaining their vital signs stable during the complicated surgery), and my fellow nurses ready, the surgery begins at 12: 00m, with an incision, right parietal tempo, with the objective of approaching the region injured by the trauma and in turn remove the hematoma, it was a hard task to manage to keep their vital signs within the margin of stability, the approach was difficult, there was much blood loss, however thanks to the work of the multidisciplinary team, The objective of stabilizing his vital signs was achieved, and with this, the hematoma was removed, and at the same time the active bleeding was stopped, which was identified at the time of the approach. This surgery lasted 5 hours and 25 minutes, and the objectives were achieved.

Luego de esa larga cirugía, de casi 6 horas, es trasladado a la unidad critica nuevamente , donde comenzaría una aventura que duraría aproximadamente 17 días, al inicio fue un duro trabajo, noches sin dormir,


After this long surgery, of almost 6 hours, he is transferred to the critical unit again, where an adventure would begin that would last approximately 17 days, at the beginning it was a hard work, nights without sleeping, whole days working hard to manage to avoid complications, and it was not only a fight on the part of us the personnel of the health, his relatives, played an important and vital role during the whole process, they were firm, solid, strong person, brave, and of a deep love, with the passage of the days for day 8 after the surgery, and previous evaluation of a new tomographic control, It is evident that his brain has improved even to the point that in medical discussions, it was decided to wake him up from the induced coma to which he was subjected for 9 days, we assumed that the work was completed, that everything had been achieved, to our surprise, after reversing the sedation and taking him out of the induced coma, he began to present problems in the ventilatory part, after 9 days with an endotracheal tube, it was impossible for him to breathe on his own, in 9 days he had received a lot of sedative medication and that had weakened his respiratory muscles, reason why it is decided to perform a small outpatient surgery called tracheostomy, this technique or procedure consists of placing a tracheostome or cannula (plastic equipment that functions as respiratory access) which is located in the trachea through an incision made in the neck which allowed the passage of air to the lungs, from that moment we began to establish and follow a protocol called, discontinuation of mechanical ventilation, which is to rotate ventilation modes, That is to say, he continued to be in contact with the artificial respirator, but with certain permissions and motivations that allowed his respiratory muscles to take strength and to begin again his physiological and natural process of breathing. This process took a little more than 4 days, and on the 13th we were able to remove the mechanical ventilator definitively, our patient was finally able to breathe on his own, without any type of problem, without any type of fear.
