Durante el ingreso del paciente a la sala de hospitalizacion la enfermera debe encargarse de valorar ciertos aspectos en el paciente, en mi servicio a parte de las funciones asistenciales tambien debemos realizar las funciones administrativas como lo es revisar la historia clinica, sacar actualizaciones, evolucionar, entre otras actividades.
Cuando el paciente ingresa lo primero que debemos hacer es revisar la historia clinica, preguntar de que servicio viene y revisar las ordenes medicas en donde se indique hospitalizar en el servicio, si el paciente llega sin historia clinica no podria ser ingresado debido a que se pueden presentar equivocaciones y de lo contrario esto podria traer problemas legales al personal de enfermeria.
Por otra parte debemos realizar un examen fisico en general como lo es conocer el diagnostico de ingreso, observar el estado de conciencia, si esta orientado o desorientado en los tres planos psicológicos, si esta respirando aire ambiente o traves de oxigeno, el aspecto físico en general como postura, marcha, observar la presencia de edema en miembros superiores e inferiores, coloración de la piel.
Por otra parte tambien debemos observar si el paciente ingresa con o sin via periferica, el interrogar al paciente y escucharlo nos permite conocer sus necesidades para asi crean un plan de cuidado especificos en base a las necesidades que presenta.
Otra de las actividades importantes es mantener la hoja de tratamiento actualizada una vez ingrese el paciente, asi mismo evolucionar su ingreso y anotar pendientes como pruebas de laboratorios, hemoderivados, control de peso y de liquidos, establecer el plan de cuidados de acuerdo a la patologia.
Las acciones de enfermeria durante toda la estadia hospitalaria contribuyen a la recuperacion y bienestar, debemos estar capacitados para atender todas las situaciones que se presenten debido a que su mejoría tambien depende de nosotros.
