Hola comunidad de Hive! Hoy es primera vez que publico sobre un tema referente a la medicina. Desde que comencé a realizar mis pasantías en el ambulatorio rural, debemos tener un ojo totalmente clínico debido que en muchos casos no contamos con exámenes complementarios que nos ayude a corroborar nuestro diagnostico o también el paciente no cuenta con los recursos para realizarlos.
En dichas guardias me ha llamado la atención que ha llegado al ambulatorio pacientes con manifestaciones cutáneas, que a simple vista podrías decir que es Dengue, pero al momento de hacer una correcto anamnesis todos la sintomatología corresponde a COVID 19.
Hello Hive community! Today is the first time that I publish on a topic related to medicine. Since I began to carry out my internships in the rural outpatient clinic, we must have a totally clinical eye because in many cases we do not have complementary tests that help us to corroborate our diagnosis or the patient does not have the resources to perform them.
In these guards, I have been struck by the fact that patients with skin manifestations have come to the outpatient clinic, which at first glance you could say is Dengue, but at the time of taking a correct anamnesis all the symptoms correspond to COVID 19.
Investigando un poco mas sobre el tema, leí un articulo publicado por Medscape el 14 de Junio de este año sobre Lesiones dermatológicas asociadas a COVID 19 en Latinoamérica en el que expresa que al principio de la pandemia no se conocía sobre las manifestaciones dermatológicas asociadas al COVID 19 , gracias al Colegio Iberoamericano de Dermatología junto a la Sociedad Mexicana de Dermatología desarrollaron un cuestionario el cual llego a un población de 700 médicos aproximadamente de los servicios y Hospitales dermatológicos de la región.
Segun el registro latinoamericano una de las manifestaciones mas frecuentes fueron el sarpullido conocido como rash maculopapular y una gran variedad de maniestaciones cutaneas como lesiones urticariformes, lesiones pápulo-vesiculares, lesiones vesiculares, seudoperniosis, lesiones papulares, equimosis, púrpura petequial, lesiones tipo pitiriasis rosada, livedo transitorio, necrosis acral, púrpura palpable, livedo racemosa y púrpura retiforme.
En cuanto a la gravedad de las manifestaciones dermatologicas el articulo destaca textualmente:
Investigating a little more on the subject, I read an article published by Medscape on June 14 of this year on *** Dermatological lesions associated with COVID 19 in Latin America *** in which it states that at the beginning of the pandemic it was not known about the dermatological manifestations associated with COVID 19, thanks to the Ibero-American College of Dermatology together with the Mexican Society of Dermatology, they developed a questionnaire which reached a population of approximately 700 doctors from dermatological services and hospitals in the region.
According to the Latin American registry, one of the most frequent manifestations was the rash known as maculopapular rash and a great variety of skin manifestations such as urticarial lesions, papulo-vesicular lesions, vesicular lesions, pseudoperniosis, papular lesions, ecchymosis, petechial purpura, lesions type pityriasis rosea , transient livedo, acral necrosis, palpable purpura, livedo racemosa, and retiform purpura.
Regarding the severity of the dermatological manifestations, the article textually highlights:
"Algunos rash o seudoperniosis se ven en pacientes que tienen respuesta inmune muy robusta y enfermedad relativamente leve. En el otro extremo del espectro, algunas manifestaciones como púrpura retiforme se presentan en la enfermedad grave, con pacientes hospitalizados", segun Freeman.
"Rash maculopapular y erupciones urticarianas lo describe que son manifestaciones de de gravedad media .
Y segun profesionales de salud destacan que las manifestaciones dermatologicas consideradas como más graves son"púrpuras, Livedo racemosa o necrosis de partes distales"
"Some rash or pseudoperniosis are seen in patients who have a very robust immune response and relatively mild disease. At the other end of the spectrum, some manifestations such as retiform purpura occur in severe disease, with hospitalized patients," according to Freeman.
"Maculopapular rash and urticarial eruptions are described as manifestations of medium severity.
And according to health professionals they highlight that the dermatological manifestations considered as more serious are "purple, Livedo racemosa or necrosis of distal parts"
Este articulo publicado por MEDSCAPE me pareció de suma importancia, ya que como personal de atención primaria de salud, papel que estoy desempeñando actualmente me ayuda a orientarme en dar un correcto diagnostico además de compartir el conocimiento a los demás compañeros de trabajo y resaltar que en esta investigación basada en evidencia arroja resultados que debemos tener presente que aproximadamente del 3 al 19% de los pacientes tenía manifestaciones cutáneas como único síntoma asociado a COVID 19.
