HBD has dimensive opportunity and we see more usage for Hive related assets in the poor or emerging countries.

Still, one thing I couldn't put my finger on, is how these people are using HIVE or HBD to pay for services: are they using an exchange or how are they bridging these assets into the real world, like building boreholes or for similar usage? If a merchant doesn't accept HIVE or HBD, how one can easily get that into money to pay for such services?

I am from Hungary. I sell Hive and Hive Dollar (HBD) for other cryptocurrencies on BlockTrades to receive these other cryptocurrencies on CoinBase, then I sell them for Euro (EUR). Then I withdraw the Euro (EUR) to my bank account. Then the bank exchange the Euro (EUR) to our local currency, the Hungarian Forint (HUF).

Probably there are similar methods in other countries too.

I was wondering the same thing.

Indeed, on- and offramping of $ to the HIVE/HBD system is the bottleneck imho. Would be a nice usage for a DHF-Fund financed project, to cooperate with a private money exchange in Venezuela or Nigeria for a possibility to on- and off-ramp $ into the system.
