I had a friend who actually did the MAID service there - he had leukemia as a kid and while he had survived it, he was left in a wheelchair and was losing function of his hands and arms too... Mind was just fine but he was unable to do anything and his quality of life had been rapidly declining so bad that he decided to end it... Which I kind of get, but his is an exceptional use case... It sounds like most folks aren't necessarily using it as intended.
