Espero que se encuentren muy bien, hoy se ha dado inicio a una nueva etapa en mi carrera de medicina, para poder optar por el título tengo que realizar el 6 meses de trabajo comunitario dedicados exclusivamente en el Ambulatorio Urbano Tipo II que se encuentra situado en un pequeño pueblo llamado Capacho, este se encuentra a 45 minutos de mi ciudad.
Hello to everyone in the OCD community! I hope you are doing very well, today is the beginning of a new stage in my medical career, in order to qualify for the degree I have to do 6 months of community work dedicated exclusively in the Type II Urban Ambulatory which is located in a small town called Capacho, this is 45 minutes away from my city.
Ya culmine el internado rotatorio en el Hospital central de San Cristóbal HCSC, tuve que pasar por cada uno de los servicios que ofrece el hospital siendo cirugía el primero en terminar hace 2 años, luego pediatría, seguí con medicina interna y terminé por Ginecobstetricia.
En estos 2 años he estudiado bastante y gracias a los doctores que hacen vida en el hospital, he logrado aprender un sin fin de técnicas, maniobras, conductas y lo más importante, un buen trato al paciente, con este conocimiento es hora de colocarlo en práctica en el área rural.
I have already completed the rotating internship at the Hospital Central de San Cristobal HCSC, I had to go through each of the services offered by the hospital being surgery the first to finish 2 years ago, then pediatrics, followed by internal medicine and ended with Gynecobstetrics.
In these 2 years I have studied a lot and thanks to the doctors who make life in the hospital, I have learned a lot of techniques, maneuvers, behaviors and most importantly, a good treatment to the patient, with this knowledge it is time to put it into practice in the rural area.
Ya que estás zonas rurales suelen tener pocos médicos y la población es realmente significativa, nos exigen asistir al ambulatoria de dos maneras, una cumpliendo el servicio diario de lunes a viernes en dónde nos encontra pasando triaje, el día de ayer tuve mi primera guardia y un caso que llegó y logramos sacar con facilidad fue un señor que presentó una lesión eritematosa en el miembro inferior derecho, el me refería que esto había aparecido súbitamente y que al ver el gran tamaño que tenía decidió asistir al centro hospitalario, una vez allí luego de interrogarlo proseguimos al examen físico y llegamos a la conclusión que nos estábamos enfrentando a una Erisipela, el problema radicaba en que está zona le dolía pero este señor es alérgico a los AINES, teniendo esto en consideración recetamos un antibiótico para combatir dicha enfermedad y algunas analgésicos no pertenecientes a la familia de los AINES.
Since these rural areas usually have few doctors and the population is really significant, we are required to attend the ambulatory in two ways, one fulfilling the daily service from Monday to Friday where we are passing triage, yesterday I had my first guard and a case that came and we managed to get out easily was a man who presented an erythematous lesion on the right lower limb, He told me that this had appeared suddenly and when he saw the large size he decided to go to the hospital, once there after questioning him we continued the physical examination and we concluded that we were facing a Erysipelas, the problem <was that this area hurt but this man is allergic to NSAIDs, taking this into consideration we prescribed an antibiotic to combat the disease and some painkillers not belonging to the family of NSAIDs.

El ambulatorio cuenta con 2 grandes características que para mí son realmente recalcables y son una poderosa planta de energía a base de Gasoil que nutre de energía a todo el ambulatorio sobretodo por los problemas con el servicio eléctrico que hemos venido presentado desde hace 2 meses para aca y por supuesto un sistema de ambulancias funcional, lo menciono sobretodo porque existen muchos ambulatorios en otros pueblitos que no poseen dicho servicio que es realmente importante e imprescindible.
The ambulatory has 2 great features that for me are really remarkable and they are a powerful power plant based on diesel fuel that supplies energy to the entire ambulatory especially because of the problems with the electric service that we have been having for 2 months now and of course a functional ambulance system, I mention this especially because there are many ambulatories in other towns that do not have this service which is really important and essential.
Es curioso pero varios de los doctores que hacen vida en este ambulatorio fueron compañeros o maestros que tuve durante mi estadia en el HCSC, actualmente son mis jefes pues ellos ya estan graduados y estan realizando el Articulo #8 para poder ejercer como medicos y optar por un post-grado aqui en Venezuela.
It is curious but several of the doctors who make life in this ambulatory were colleagues or teachers I had during my stay in the HCSC, they are currently my bosses because they are already graduated and are making the Article # 8 to be able to practice as doctors and opt for a post-graduate degree here in Venezuela.

Luego de una guardia bastante atareada, por la mañana a las 6am nos tocó ir de post-guardia (Si dormimos, de 2:30am a 5:30am) a una jornada en una comunidad que es de bajos recursos, junto a UNICEF nos embarcamos y logramos atender distintas necesidades en este viaje.
Ha Sido una grandiosa experiencia y a partir de hoy es oficialmente mi inicio como laborador en el área de medicina.
After a busy shift, in the morning at 6am we had to go post-guard (we managed to sleep for a while, from 2:30am to 5:30am) to a journey in a community that is low-income, along with UNICEF we embarked and managed to meet various needs on this trip.
It has been a great experience and as of today is officially my start as a medical laborer.
