Woaah same situation with my nephew. Her mom got pregnant at the age of 14. Her mom, my sister uses my father's middle name instead of sur name and used it as her sur name in the birth certificate/ live birth of my nephew. And it was only discovered when he processed his passport as you did. Now he can't apply abroad unless everything will be settled. Worst thing is, he used his father's sur name in all his identification cards and school records. I hope everything you're going through will be paid off with good result.
