I have pondered the etiquette of responses here on Hive a time or two. My thoughts are similar to yours. I settled on the conclusion that most times one of the two is sufficient, a vote or comment. My reasoning is simple, both indicate that the comment was read. The Vote given don't even have to register for rewards. Its the indication that it was read that is most important in regards to etiquette. There's exceptions as you pointed out. A full post addressing many as you just did. Not interacting with a regular 1 or 2 times out of ten is acceptable in my eyes. Sometimes things just don't need to be interacted with.
