Unless your phone is necessary for your job, put it away while working. 30 minute blocks of work followed by rewards help get items on that to do list done.

I just use notepad, write down a bunch of stuff I'd like to accomplish for the day, and separate completed tasks from uncompleted ones by a space. I generally list them how I'd like to finish them chronologically and include the rewards on the to do list to have some easy wins. For instance it will look something like this:
-make bed
-10 minute stretch
-make breakfast, take vitamins
-walk dog
-shower
-read through steemit feed, comment, read steemit.chat links respond
-snack
-60 minutes writing
-rock climbing
etc.

I'll give Moleskine a look, could use a planner for longer projects that I can't finish quickly, remembering birthdays, and other long term things I may forget with my notepad method.

And absolutely love "living in the moment" and "dont be afraid to say no".
