My default is always to play the cards/situations, but if I have strong reason to believe someone is doing something very far from optimal or is exploitable in some way I'll deviate my strategy.

For instance, let's say a standard player shows up in a spot with 20% of hands, but someone is playing crazy and will show up with 50% of hands instead. I have to adjust my play against this opponent because it would be silly not to. I guess what I should say is, the better the players are the more I play the cards and the worse the players are the more I deviate, because the worse players are generally making large mistakes that can be exploited in certain ways.
