I would consider at notice at the top of the boxes on the main page saying "Online" or "Online/In-Store" with the city listed, for example:

(SBD only, Online/In-Store)

Tim Cliff's Cliff Diving
100 Murder Lane,
Englewood Cliffs, NJ


(STEEM/SBD In-Store)

Acidyo Head Shop
666 Deadhead Way,
Knawbone, KY
