I think it safe to say that. the arrangement of medical history would depend on various factors. Some people were taught in a different way from others but I think we can all agree that all of them serve the same purpose; to achieve an accurate provisional diagnosis with a few differentials.

Indeed, the format may change, but what matters is that they all have the same detailed information about the patient's case for easy reference
