Trastuzumab, is a monoclonal antibody which targets HER2 specifically (its actually the blue portion of the structure in my thumbnail image). Neratinib is an active site binding small molecule, which inhibits the phosphorylation activity of the HER kinases, IE its a pan inhibitor, it can hit more than just HER2 while Trastuzumab is specific for just the one enzyme. I do not know about its side effects, this article was more focusing on the use of the inhibitor to generate information about the various mutant forms of the enzymes, and where the inhibitor was able to generate a positive effect. It wasn't a deep dive into the pharmacokinetics and off target activities of neratinib.
