I can recreate this bug by switching back and forth b/w plaintext markdown editor and WYSIWYG editor. It's a typical bug of crappy WYSIWYG editors. Not sure why it works this way.
I suggest you only use plaintext editor and submit a bug report to Steemit devs, if there isn't already.
