As I mentioned in one our previous conversations, the translation suggested by you changes the meaning of the sentence :) In my opinion "no matter how small" refers to the seriousness of a decision and not the seriousness of anxiety. So in my opinion - regardless of whether it sounds better or not - it changes the meaning of the original sentence. I remember what you said, that you don't like to translate literally, and it's fine, but a new meaning to what was said should not be added I guess.
