Some time ago there was an accident like this in a cave in Ushuaia, Tierra del Fuego and yesterday another accident in a frozen cave in El Bolsón, Rio Negro (Argentina), both with fatalities. Very sad :(.
This Ice cave is regularly controlled and the ice movement is checked. But yeah, maybe it is never completely safe. In this cave you can only go with a tour guide. ;)Â

That's why I'm afraid to enter caves with glaciers... Prone to accidents..
