Push ups determine your overall strength.. i think its one of few exercises which indulges your whole body. Try all type of pushups, Diamond pushups, closed grip push, wide grip, elevated type and bench let raised types... These all are monstrous ;)

Yeah, switching it up is a good idea. Works out more overall points.
I feel terrible pain when I try it. When I try sit ups it's even worse, any suggestions on what to do?

First consult with your doctor. After that try to do some back exercises without weight.
